Safety Guidance and Procedures
There has been a recent accident where the key hole of a heavy lidded “reinforced” JF4 fractured and broke whilst the lid was being put back into the frame (photos below), causing the engineer to fall backwards and hurt himself resulting in a painful injury, and time off of work.

 

When opening a joint box please remember to do the following as well as completing all your normal checks and procedures:-

 

·         Guard the area appropriately

·         Always use your roller bar when removing and replacing lids

·         Carefully inspect the lid for any damage or signs of stress or fractures, particularly around the key hole area before attempting to lift or remove the lid

·         Ensure your lifter and counterlever mechanism is in good condition

·         Always perform a full gas test, and continually monitor your breathing zone with your GDU
